The next step in preparing for the PBGC appeal process is that you must complete a Designation of Representative Form (more commonly called a Power of Attorney form.) By filling out this form you give our attorneys the power to represent you before the PBGC in its appeals process. This designation of representation is separate and different from the proxy you gave DP3 during membership enrollment. Please follow these steps to complete the administrative preparation for the PBGC appeals:

2. Sign and date the form, and have your signature witnessed by another person (it does not have to be notarized and it may be witnessed by your spouse.)
3. Send the completed form to DP3 by one of these means:
   • Scan the completed form to a PDF and email it to DP3 as an attachment.
   • Fax to DP3 at 888-505-1242.
   • Mail to DP3 at DP3, Inc., 8014 Cumming Hwy, Ste 403-333, Canton, GA 30115

Please note that due to the volume of survey responses and faxes, we may not be able to individually acknowledge receipt of individual surveys or forms. However, we will periodically report the receipt of the both in our email message traffic in a manner similar to the Membership Status Updates we have been using on our recent emails.
